Perform a thorough, full printer inspection for paper or paper pieces stuck inside. Do not neglect to check between the upper and lower paper feed guides (remove tray 2 cassette and press the green button at the front of the printer).
Replace worn pickup and separation rollers. Use only OEM or high quality compatible rollers.
Pickup roller not installed properly – Ensure the tab/hook on top of the pickup roller is properly mounted/hanging on the sheet metal on the pickup assembly.
Ensure cassette guides are properly aligned/snug against paper stack.
Replace tray 2 pickup assembly.
